HolyWave Offering Number Request Form

All HolyWave and SRCC members who have completed the BELONG or SRCC membership course can use their SRCC membership numbers to request for a tax statement at the end of the year.

If you are NOT a HolyWave or SRCC member and would like an Offering Number, please proceed with completing this request form. The Offering Number will help our administrators accurately log your giving history and this will help with pulling up an accurate account when you request for your tax statement at the end of each year.

You will receive an Offering Number and instructions once we process your request.
